Why is my 2011 Toyota Sienna blowing hot air when fan (not A/C) is set to cool?

Why is my 2011 Toyota Sienna blowing hot air when fan (not A/C) is set to
cool? With outside air temperature in high 60's and windows down, fan is
blowing air that feels 15-20 degrees hotter. This is right after starting the
engine too.
The cooling intensity of the A/C seems to have been weakening over the last
few months too. But this new issue cropped up suddenly.
