1992 gmc sierra 4.3 losses power in 5th & 4th gear

no back fire, but loses pwr in higher gears. ok if I shift down

most likely fuel pump or pressure issue. high gear at highway speeds while trying to slowly accelerate is a high load condition-pump may not be able to handle the demand.
