Should I ever pay higher than a Blue Book price range?

Car lots often charge higher than Kelly Blue Book which doesn't seem right.  
How do you know when it is okay to pay over Blue Book pricing?  Has Blue
Book pricing obsolete?

KBB always seems to be on the low end. I can check NADA on the exact same vehicle and its always a big difference in pricing.
