Car not picking up and is jerking

Usually my car has great pick up, but lately
whenever I press the gas pedal it barely gets up to
20mi/hr and that's with my foot pressing pretty
hard. Not only does it take me forever to get me up
to 40mi/hr, my car jerks back as I accelerate! What
is happening??

What is model year? Problem at higher rpms, the basics, you check for a blockage of some kind, air--fuel--exhaust, something of that nature. How long since a tune-up? Any applicable trouble codes?
