When I put my 98 Honda Accord into reverse it
makes a horrible grinding noise. Is it the
transmission gear box...

You need to change transmission fluid and filter then take for a 20 to 30 mile drive and should be good.usually that happens because dirty transmission oil or needs transmission oil one of the 2 check the trans dipstick.

Try and shift from 1st to reverse without letting the clutch out
