Does this need to be fixed?

This the left passenger side.  When the metal collapsed, a lot of water came
out.  How serious is this and what is the component called?

That is bad and I would bet there is a lot more rust to be found. Probably time to dump this Jeep.

F_O_R is right, but for now pump it full of expanding foam to seal and strengthen it and drive it until the motor quits. If you have the six cylinder engine, that will be a long time. That looks like the lower rear quarter panel that filled up with water and rusted out. Rocker panels do that too.

If you live in a state that has inspections you have a problem. Take it to a body shop if you are brave enough!

Would you use low expandable or the high expandable foam? I was also thinking of treating it with a rust inhibitor to slow the decay. It's a 6. It would be great if I could squeeze another year or 2 out of this car.

I think I used low expansion foam but I'm not sure. You'll be working upside down so be ready to control the drip and sagging with a piece of cardboard until it hardens. And start off with the area clean and dry so it will bond. Later on you can apply an oily rust inhibitor and it will not react with the foam once it hardens. Good Luck my friend.
