While driving the rear starts to shake and thump like it has a flat tire.

It goes away when I slow down and seems to come on randomly meaning its not always doing this. Could this be the universal joint? It feels like the rear tire had a flat and there is a thumping coming from the back of the truck.  I

Sounds like a out of round tire (slipped belt in tire) run your hand across the tread of rear tires if you feel a bulge the tire has a slipped belt. But also check universal joints

also check the tires for balance and if the lug nuts on the wheel are loose. Might also be a worn shock as well.

The passenger side rear tire looks pretty bad and may be the issue going to chacnge it since it needs it anyway, then I'll check the U-joints and Shocks. Thanks
