Bad blend door actuator. Blend doors sit on the heater core box, some are easier to get to than others. (some take removing dash to swap out, some can be reached through glove box opening or air bag opening.) Look up your blend door location and proceed from there, most dedicated forums will have a post or two on how to replace them.
