Chopping Noise from Rear Tires

My car has been making some sort of
chopping noise from the rear tires for quite a
while, but it's been getting a lot louder lately.
The noise happens when I'm driving, slowing
down, and when I turn. Whenever I get to a
faster speed, such as on the interstate, it will
no longer make that noise, but if I stay on the
local roads it will.  The rear driver side is
making the most noise. My brake pads are
very worn down and my rotor has a few
grooves. Is it possibly the brakes could be
making this chopping, slightly grinding
noise?

i just replaced my shocks and just discovered that my rear driver tire is turned inwards towards the top. does that change anything?
