Ford windstar 3.0 keeps loses power then dies. wait ...and starts back up????

Almost seems like isn't getting enough gas.  Does lose rpms.  Have changed
fuel filter.  Runs good for days.  then out of blue....dies.  wait an hour and it
starts back up and runs.  Have put in some seafoam and high octane gas.  
same results.

I would check the following clean your throttle body good with a good Cleaner such as CRC, check the throttle body sensor, check you crankcase sensor, check your pump pressure ..
