I have a 2002 5.9, can't get the bolt off of the back of my tensioner, what size bolt is that and is it reverse thread? (since the bolt is mounted on the tensioner)

If I am not mistaken, the tensioner is removed from the engine and thrown away. Then put a new one on and you're done.
