Can I just swap a set of 16 inch 2000 crown
Vic rims to a 1995 crown Vic with 15 inch
rims?

Both the 2000 and 1995 have a 12mm offset so in general the rims should fit. The overall height of the well will change depending on the tires mounted on them. The OEM size of 225/60R16 should work without any rubbing issues.
