I had the same problem and fixed it finally today. The hot water inlet hose coming from the engine's intake manifold is connected with a "quick connect" fitting. The hole that carries water through that fitting is quite small. Maybe 1/4 inch. On my truck, debris (old gasket material, etc.) was carried to the fitting and stuck there. Blocking water from flowing through the heater core. I replaced the fitting with a Edelbrock 8184, which eliminates that odd quick-connect and associated special hose, and provides a nice larger 1/2 inch passage for water. Now the heater core outlet hose going to the radiator gets hot and my cab gets hot.
