What are the possible cause of 3 out of 5 lug nuts missing on a tire?

I noticed a familiar sound/ vibration
while I was driving before seeing 3
adjacent lug nuts missing on my front
passenger tire. Two adjacent lug nuts
were still on. This happened to me
previously about a year ago (same car
and same location: front passenger
side) except 1 of my studs was
snapped off as well, so I've had a new
stud and new set of lugnuts put in
since then. What could be possible
causes of this?

Lug nuts do have a specific setting they should be torqued to.

the most common reason for wheel becoming loose is corrosion buildup between the wheel and the hub. This prevents the wheel from properly seating onto the hub. Even if the lug nuts are torqued correctly the wheel eventually comes loose. During vehicle operation the lug nut back off and the studs can break. This can actually occur days, weeks, months after the wheel was originally installed.
