Mine was on a wallet card with the manual but it's also marked on the computer module. Page 64 in the owner's manual explains how to program your own code once you find the original one. If you can't find the original code, you'll have to take it to a dealership.

I have a 2003 sport trac. I personally did this maybe 6 weeks ago. You will have to gently remove the upper top part of the dash. ( sounds worse than it is) a screwdriver will help. Be gentle it clips in & off the dash. When that is removed, get a flash light stand outside tbe truck, on ghe front passanger side looking in thru the windshield. Look rite into the hole where the dash pad had been, you should see a yellowish/beige plastic piece shine your lite on it. Tbere should be 5 numbers thst are all odd numbers. Those are the magic numbers. Im not mechanical at all took me all of maybe 5-6.min to take apart dash find numbers reassamble dash and make sure code worked. Very easy. Good luck.
