Transmission problem

Sometimes truck will rev up but won't gonna  
over 15mph. Was told to replace
transmission filter but not fluid. But that
doing so I'd still have to replace
transmission very soon

To replace the filter, you have to drop the pan, which then you will be removing and changing the fluid. When was the last time you serviced the transmission? Not going over 15mph isn't fluid or filter related, IMO. On a 27 year old truck, the odds of others transmission internals that are worn are high.
