2002 grand caravan runs rough when heated up and accelerating

Car will drive fine for maybe 1/2 hr then starts to miss and jerk, loses power. Run ok little
bit and just keeps getting worse the more you drive. Will drive ok on maybe 1/3 throttle,
but if you need to go up a hill or accelerate much, it bogs down, misses, etc.
