Ford Escape Jolting Feeling

While accelerating slowly it feels like my Ford Escape starts to joltat around 45
miles per hour when the rpms are around 1900.  The feeling is similar to
when you shift up too early on a manual transmission and you need to speed
up so it doesn't die.  It happens right when it shifts down from around 2500
rpms to 1900 rpms.   It will continue off and on until I get above 65 miles per
hour.  If I accelerate quickly and it doesn't have a chance to down shift to the
1900 rpms it does not do this at all.  If I'm driving around 30 miles per hour at
the same 19000 rpms there is no jolting feeling.  One mechanic that I drove
around with thought that it might be the PCM, recently replace, not
communicating with TCM correctly. There are no lights light up on the
dashboard. Any thoughts?

Some additional information: My Escape is a 2005 with 174k miles, 4WD.
