2001 hyundai accent l 1,5 liter engine CHECK ENGINE LIGHT WONT GO OFF

I have a 2001 Hyundai Accent L 1.5 liter engine  in  Las Vegas ,Nevada
NEVER ANY PROBLEMS with this car
Till went to smog it this year 2014
2 codes are  Catalyst and Evap   from SMOG PLACE
went to Auto Zone had diagnostics done
it says
Engine Running Lean  
Fuel Filter and or Pump
Vacuum Leak
Engine Problems
we cant find the problem  changed fuel filter,spark plugs, injection cleaning etc.
check engine light WILL NOT GO OFF
PLEASE HELP   
If anyone has some advice  or is familiar with this problem
No other vehicle
so on the bus

Based on these readouts.. Engine Running Lean -Vacuum Leak-Engine Problems ...You have a Vacuum leak. I have used a windex type sprayer with plain water Here is a shabby video but it should give you the basics https://www.youtube.com/watch?v=MI2o9YNZ2Y0

Hi thanks for info - we tried no vacuum leak. Went to Dmv . Emission specislist there put car on machine. He says the code that is coming up is. P1128 Take to shop and fix . What does that code mean and what should be done to fix car?

This code is usually caused by a failing mass airflow sensor which would also cause poor gas mileage.

The MAF is usually located in between the Air Filter and the throttle
