2005 ford freestyle why is my wrench light on

My wrench light is on and won't turn off
and I have almost no acceleration and
it won't rev past 4000 rpm's

The wrench light means it needs maintenance. This is in the owner's manual, if you don't have one you can often download them for free through the car maker's web site.
