Tranny won't shift out of first when driving

There are several factors but the 3 main ones I am seeing are that the transmission governor has gone bad, the solenoids may need replaced, or the entire transmission has just finally kicked the bucket. If your truck has an electronically controlled transmission it can also be a blown fuse. I would try the transmission governor first, they are usually around 15 dollars.
