Transmission don't like to driven low on fluid as heat builds up burning the clutches. You might need a rebuild or your still low on fluid. Repair the cooler hose first before diving into the transmission. Why is it popping off, is the connection stripped?

Did you fill the transmission with the engine off, then pull the stick? If you did, your tranny is not full. You have to pull the stick with the engine running. Pull the stick with engine running, wipe stick, put back in and get reading. Then add more oil as needed. Most people check the tranny fluid like they check their oil. It will appear full when cold and the engine is off.
