99 Toyota Camry ce fans by radiator only turn on when inside knob is turned to defrost. Is that normal

I don't think you have anything to worry about, as long as it's not overheating. The fan comes on because when you turn defrost on, the A/C automatically kicks on to de-humidify, and therefore the fan will also. And while driving, there is enough air-flow through grille to make fans redundant or useless, so they don't come on. As for not seeing water-flow, I really don't know what to say about that, as long as gauge is in the middle, you're good to go.
