Is it common for the radiator hose to fall

No, it is not common for it to fall off. It sounds like it's missing a clamp to hold it in place. Clamps come in two types -- a spring loaded style and a screw-down style. The most common problem with a radiator hose is that it gets soft and weak over time and ends up splitting open.
