Rear end drive shaft cuppling

I took off the driveshaft because the coupling fell off from the read end.  I
replaced it with a new coupling, but it keeps falling off.  I am unsure of the
reason it does not stay attached.  Can you please give me suggestions on
repairs?

I would take it to a shop who knows how to tighten this nut. There is a pinion back lash that has to be set while tightening this nut. If it is not followed, you will create problems with the differential ring and pinion gear. The crush bushing may already need replacing from you tightening the nut. There is a specific procedure to follow when tightening this nut. You may need a new nut and locktite applied to the threads. Best take it to someone who knows how to do this.

Is the nut holding the pinion flange coming loose or are the u-joint retaining fasteners coming loose?
