I would suspect something inside the transmission to be faulty. First thing to do is pull the transmission pan and see what parts may be sitting in it. Small metal flakes are normal. Inspect the reverse band and check for a broken piston or spring. If you see metal pieces then you need to have the transmission rebuilt. But another thing is to check your shifter cable adjustment.
