1990 Ford 5 speed manual transmission

What would make my manual transmission
appear to be e neutral when gears shift
correctly when engine is running and when it
isn't?

No linkage on the manual transmission and wouldn't the truck try to lunge forward if trying to start in gear even if the clutch is bad?
