My 1969 Beetle convertible starts shaking, gearshift and body, in 4th gear at about
45 miles an hour.  Runs well at lower speeds and gears.

Check your wheel balance and tires, alignment and anything loose in the suspension and frame. Lug nuts nice and tight?

Yep, broken belt in a tire, bent rim, axle bearings loose, stuff like that. Good call on the lug nuts Tom
