serpentine belt came off how do you put it back on

Belt came off how do you replace it?  What would cause it to come off?

look for worn spots on the belt replace if need and make sure all pulleys are moving freely as they should and check your tensioner to make sure it is moving back and forth to tighten belt like its suppose to be...
