When did you service it last? What is the fluids color?

The fluid is kinda old it needs change but could that be all of the reason

Could be the shift solenoid http://www.ehow.com/how-does_5050409_solenoid-dodge-transmission.html

If the filter is plugged up from varnished and or burnt fluid it can cause what you are experiencing, I soul suggest dropping the trans pan and check the color and smell, if it is dark and smells burnt then your tranny isn't going to work correctly.
