Some block heaters screw in - some have a machine screw that pulls a flat bar or arms to hold it in place. There is a rubber gasket or O ring that makes it watertight. http://www.warehouseautoparts.com/Specialty_Line/Kat/images/Kat's%20 _10435-04.JPG

There's many different types of heaters. Some are non invasive like the dip stick heater or the stick on oil pan heater. Others go in a coolant line like Jimmy pointed out. There's even car battery heaters to get more cranking amps. --- The downside of a block heater is that they eventually corrode and leak after ten years or so..
